[Solved] Unpivot How to

Hellos, 

 I am playing with unpivot and throwing it' test data , but cannot figure what is possible with unpivot here.   I have attached two possible output 

If you can help me figure this , it would be great

It is very similar to the one one will find in the wiki

 

I kept messing up the Output field. Thank you for detailed output .I was able to play and get my second output as well.

I will now try few more experiments and revert back (with MORE MORE MORE questions)
