Question for import csv file with header and trailer

Hi Team, 
Using Advanced ETL processor ENT 6.4.2.17.
We will receive a csv file in the following format.
The first 2 rows are the header and the last 2 rows are the trailer, we need to import the rows in the between of header and trailer into a table of MSSQL database.
Tried to use the following setting but no records can be displayed.

Lines to Skip
Header = 2
Footer = 2
How to import this csv file.

Please refer to the attached dump screens, csv file and vsi file.

You are using incorrect line terminator it should be {LF}
