Error Message running Transform Object

Attached is a screenshot when I attempt to run the Transform Object (Error.png)
It states that in my WRITER output TOLLFREE Nothing Is Mapped and an access violation message.

Attached is a screenshot of the Validator showing my mapped fields (Validation.png)

Attached is a screenshot of the WRITER, EXCEL FILE tab (writer.png)

When I attempt to run it again, I get another error message (error1.png)

The only way I can get rid of the messages is restarting the ETL program again.

I am not sure why it cant find my mapped columns going to my writer.

I appreciate any insights.

I was able to finally resolve this. I had to recreated it and finally the errors went away. I am not sure what had happened.
